- Description: Universal ER suicide screening catches more people at risk. MB investigates what happens or doesn't after that positive screen.

Universal screening found more at-risk patients. It didn't guarantee they got care.

## What the Research Found

Research using ED-SAFE data found that patients with an “incidental” positive suicide-risk screen — flagged during a visit for something unrelated, like a broken bone or chest pain — had post-visit suicide-related outcomes similar to patients whose screen matched their reason for the visit. But the incidental group was far less likely to receive a follow-up assessment or intervention. Universal screening did improve identification of non-white patients specifically.

## The Gap This Exposes

Better detection without matched follow-through can produce a paper trail of risk without an actual intervention behind it. A positive screen that goes nowhere isn't a safety net it's documentation.

## The Accountability Question

Does an ER required to screen every patient also have the staff, beds, and same-day psychiatric consult access to act on a positive result or is screening a policy sitting on top of a system that was never built to respond to it? That's a hospital-by-hospital, budget-by-budget question, and it's answerable with public records.
